Broken Graphics Card

Hey all, I am having a problem with my NVIDIA GeForce 8400 GS graphic card… There are streaks, dots, boxes, ect, running across my screen as I turn on the power for the computer. I tested both the monitor and the PC on 2 seperate PCs/monitors, and it seems to be a problem with my PC only, so I am led to believe it is my graphics card.
